The Care Home Navigator Scheme is a 12-month project to support family members through the process when loved ones are moved into long term care settings.
We understand the difficulties families face when the decision is made to move a loved one into care. It’s not a decision families take lightly, and this can cause a lot of stress and anxiety.
Our role is to support unpaid carers who may have been providing care for many years to be supported throughout the process and ensure carers have all the information they need.
We can support unpaid carers with information, advice and guidance, signposting to other relevant organisations, provide carer involvement forums, access to respite and breaks, access to breaks, complimentary therapies, support with education, employment and volunteering and a network of peer-to-peer support across Hartlepool.
